Procedure Prep Instructions
At the time your procedure is scheduled, a member of our staff will give you specific procedure preparation instructions to follow. Below is a listing of the various procedure preparations used by our physicians. If you are uncertain as to which document applies to your procedure, please contact our office.

Following are a few things that you, as a new patient, will need to do prior to or at the time you first receive care from our physicians.
- Understand the extent of your insurance coverage for office visits and any procedures that you are anticipating in the course of your care with our physicians.
- Review our Insurance and Billing policies and Notice of Privacy Practices
- Complete our New Patient Forms
- Bring your driver’s license and your primary or secondary insurance cards.
- Bring a list of all your current prescription medications
During your initial visit we will collect as much information as possible. However, when you return to our office or come to our endoscopy center, it is important that you always bring your current insurance cards and driver’s license. Please be sure to update us regarding any changes of address or phone number.
Our office is open between the hours of 8:00 a.m. and 5:00 p.m. Monday through Friday (excluding holidays) and closed on the weekends.
The most convenient way to refill a prescription is to contact your pharmacy directly. In most cases the process is automated and only requires the information contained on the label of your prescription bottle. Even if you have no more refills, your pharmacy can contact our office to renew your prescription. If additional information is needed, someone from our office will contact you to discuss your need for additional medication or arrange an office visit to ensure proper treatment.
If preferred, you can request a refill by contacting our office at (812)477-6103. When calling your office, you have two options: you can use our convenient automated refill line or you can leave a message regarding your refill request. Refill requests received after 12 noon will be processed the next business day.
Prescriptions for narcotics and pain medications will be filled only during office hours, Monday – Friday and not after hours or on weekends.
If you have a medical emergency, please seek emergency assistance by dialing 9-1-1. For non-emergent matters, a gastroenterologist is on-call 24 hours a day, 7 days a week. Outside of our normal business hours, the on-call physician can be reached by calling (812) 477-6103. An operator will take your call and forward your message to the on-call physician who will later contact you.
Outpatient Procedure Rescheduling/Cancellation
We make every effort to strive for excellence in our specialty and in our profession. An important aspect of providing quality care is to be respectful of your time and make every attempt to accommodate your scheduling needs. To accomplish this, it is very important to be on time for your outpatient procedure, arriving early for paperwork, and to notify us in the event you need to reschedule your outpatient procedure. We reserve an ample amount of time for our patients because we feel it is important in providing quality health care. Therefore, sufficient notice to change your outpatient procedure appointment is necessary in order to adjust staffing and to have the opportunity to offer this time to another patient.
We greatly appreciate your consideration in calling us at least 24 hours prior to your scheduled outpatient procedure for any cancellations or rescheduling needs.
Outpatient Procedure Appointments that are cancelled/rescheduled without sufficient notice (24 hours) will incur a charge of $50.00. It is our sincere hope that this will not be necessary.
Please Note: This charge is not covered or paid by any Insurance Company, therefore the charge will be billed directly to the patient.
